New Patient Forms
Save time at your first appointment by filling out your new patient form and photography consent online ahead of time. Arrive 15–20 minutes early if you'd rather fill them out in the office.
Insurance & Payment
We're in-network with most major PPO carriers and will verify your benefits before treatment begins, so there are no surprises on your bill. No insurance? We accept cash, all major credit cards, and CareCredit financing for larger treatment plans.

Frequently asked questions
Do you accept my insurance?
We're in-network with most major carriers, including Delta Dental, Cigna, MetLife, Aetna, and Guardian. Call our office and we'll verify your specific plan before your visit.
What if I don't have insurance?
We offer transparent, up-front pricing and can discuss payment options for patients without coverage. Free consultations are available for Invisalign, emergencies, and second opinions.
How early should I arrive as a new patient?
Please arrive 15 minutes early with your completed new patient forms and a photo ID and insurance card, if applicable.
Do you offer sedation for anxious patients?
Yes - nitrous oxide and oral conscious sedation are both available. Mention your anxiety when booking so we can prepare accordingly.
Do you have Saturday appointments?
Yes - we're open Saturdays from 9:00 AM to 1:00 PM for patients who can't take time off during the week, offering the same routine and emergency care available on weekdays.
Can you see me the same day for a toothache or dental emergency?
We do our best to see emergency cases the same day, including toothaches, lost fillings or crowns, and dental trauma. Call our office as soon as symptoms start.
Do you treat children as well as adults?
Yes - Smilecraft Dental welcomes patients of all ages, from a child's first checkup through adult and senior care.
How much does Invisalign cost, and is there a current offer?
Invisalign cost depends on case complexity and is confirmed at a free consultation. We currently offer New patients: free exam & X-rays - ask our team when you book.
What should I bring to my first appointment?
Bring a photo ID, your insurance card (if applicable), and a list of any current medications. Completing your new patient forms online beforehand saves time in the office.
